Centra’s Remote Home Monitoring Program Enhances Patient Care Through Technology

Centra has taken a significant leap in furthering its strategic theme of One Centra with the launch of its Remote Home Monitoring (RHM) Program.

This innovative initiative, conceived during the challenges of the COVID-19 pandemic, aims to extend care directly to patients' homes and redefine the patient experience. The RHM Program is designed to reduce hospital readmissions and emergency department visits while helping patients manage chronic conditions such as diabetes, congestive heart failure, pneumonia, COPD and hypertension.

Referred patients who opt to participate receive a comprehensive kit including a tablet, digital scale, blood pressure cuff, pulse oximeter and heart rate monitor. These devices connect via Bluetooth to the tablet, which automatically transmits data to Centra's Command Center Team for continuous monitoring. The tablets also remind patients to take their medications and complete daily surveys to assess their well-being.

Feedback from participants has been overwhelmingly positive, with patients appreciating the additional support provided through the program. The average “length of stay” in the program ranges from 48 to 90 days, emphasizing proactive care and continuous support to help patients establish better health habits.

Tab Sprouse, Centra’s Senior Director of Transport Services and the Call Center, highlighted the positive impact of the RHM Program, stating, “Patients are asked on their tablet surveys, ‘My experience with the service has been positive.’ 60% of respondents have said they strongly agree, and 40% agree, with none answering neutral or lower.”

The RHM Program exemplifies Centra’s strategic theme of One Centra, promoting a unified approach to service standards, resource allocation and healthcare delivery. The program has successfully served patients from the Program of All-Inclusive Care for the Elderly (PACE), CMG Offices and those who received care at one of Centra’s hospitals.

The launch and continued success of the RHM Program have clearly demonstrated its value in enhancing patient care and supporting the One Centra vision of integrated and patient-centered healthcare.
