Below are three surveys from Centra Health School of Practical Nursing. Their purpose is to evaluate the program of study and improve the curriculum. All information collected will remain confidential.

Only graduates who have received letters of instruction should fill out these forms.

The One Year Follow-Up Study is to be completed by individuals who graduated from the School of Nursing within the last 18 months.

The Employer Follow-Up Study is to be completed by the employer of a graduate who completed the program of study within the last 18 months.

The Five Year Follow-Up Survey is to be completed by graduates who completed the program of study five years ago.
